Abstract
The integrated patient development record is a form that must be enshrined. This sheet must be signed by the treating doctor. Based on a preliminary survey at the Asembagus Hospital of Situbondo Regency, in filling in the integrated patient development record forms in inpatients 70 medical record documents found incomplete filling in the form on item No. RM, patient identity, date of MRS, SOAP, name of DPJP, name of registrar. The purpose of this study was to identify the factors that caused the incompleteness of integrated patient development records at the Asembagus General Hospital, Situbondo Regency. This type of research is qualitative, the subjects in this study consisted of 4 people, namely doctors, heads of medical records and nurses. The sample used was 70 medical records from the total population. The way to collect data is by observation, interview and documentation. The results obtained in this study are the first priority of medical personnel who have never attended training on filling out medical resumes so that efforts are held to hold training and seminars to make officers better understand the importance of completeness of medical resumes considering that not all officers weigh all behind the medical record.
