Abstract

Background: Non-hemorrhagic stroke (NHS) is one of the leading causes of disability and death worldwide, often followed by complications such as limb weakness (hemiparesis), chewing and swallowing difficulties, and impaired nutritional status. Objective: To provide standardized clinical nutrition care for an inpatient diagnosed with Right Hemiparesis due to NonHemorrhagic Stroke. Methods: A case study was conducted on Mr. J.I., an 87-year-old male, presenting with weakness on the right side of the body, headache, fever, and cough, with a history of hypertension. Nutritional assessment included anthropometric, biochemical, and physical/clinical data, and a three-day dietary recall, followed by nutrition diagnosis, intervention, and monitoring and evaluation using the Nutrition Care Process (NCP). Results: The patient showed undernutrition (MUAC percentage of 81.4%), elevated LDL cholesterol (155 mg/dL) and triglycerides (244 mg/dL), high blood pressure and body temperature, and chewing/swallowing difficulty that resulted in a severe intake deficit (average below 50% of requirements) for energy and macronutrients. The intervention consisted of a soft-textured, lowfat, low-sodium stroke diet providing 1,783.37 kcal, 56 g protein, 49.53 g fat, and 278.38 g carbohydrate daily, accompanied by nutrition education for the patient and family. Conclusion: The patient's main nutritional problems were related to dysphagia caused by the stroke and previously unhealthy eating habits, requiring dietary modification and ongoing education to support recovery.
