Abstract

Medical Record System is a series of several systems that exist in the process of organizing medical records. One medical record file is a general consent form. This is the participation of the patient in charge to participate in performing the medical record properly and correctly including by filling out the complete and proper medical record form, but the lack of knowledge of the patient responsible regarding the importance of medical record can harm the family / patient. This research uses quantitative approach with Cross Sectinal. Data were obtained from checklists and questionnaires filled out by the person in charge of the patient. In this research, there were significant univariate analysis for the highest age characteristic, 36-45 years old as many as 35 respondents, gender more that is male with 49 respondents, highest level of education is SMA 33 respondents. Based on the most work is 64 respondents work. The level of knowledge is approximately 35 respondents (44.3%). Level of attitude that does not support more that is 49 respondents (62%). While the result of bivariate shows the significance value of knowledge relation with filling of general consent is 0,000 (p <0,05) and significance value for attitude relationship with general consent filling is 0,040 (p <0,05). It is advisable for the hospital, especially the registration section to provide good communication and re-understanding to the responsible patient in relation to the general consent, because a good understanding will affect the knowledge and attitude of the patient in charge of general consent in Balung Regional Hospital.
