Abstract
The case of patient health services that do not meet the minimum service standards occurs in the process of taking medical record files which takes less than 10 minutes and difficulties in the process of searching for patient data in the register book or main patient index card when the patient does not carry an index card for treatment, this is It can be seen by the observation technique of the rules governing visitors caused by the large number patients visiting every day, reaching a number of approximately tens or even hundreds patients. Various cases can also be encountered such as in borrowing and receiving medical record documents, with many patients who have visited the medical record staff who recorded loans and medical record documents who were overwhelmed so that the slow service of medical record files and missing medical record files could occur. or misfile, the purpose of this study was create an information system that can help officers who handle the Nangkaan Bondowoso Community Health Center. This type of qualitative research uses observation data techniques, interviews, documentation, and brainstorming. Method used prototype method, with prototype research method and user can solve problems and find solutions by using interview and brainstorming techniques to 5 informants, namely the head the puskesmas, doctors, medical records officers, registration officers, and operator admin.
