Abstract

Regional Hospital dr. Soebandi Jember in improving the quality of inpatient care AKLPCM have AKLPCM standard of <2% each month, in the last two years the average value AKLPCM patient wards every month was 10.5% in the year 2014 and amounted to 15.13% by 2015, yet AKLPCM meet the standards that have been set. The purpose of this study was to analyze the causes AKLPCM the wards in medical records Regional Hospital dr. Soebandi Jember with variable Man, Material, Machine, Method, Money using fishbone diagrams. This study is a qualitative research interviews, observation and brianstroming. Results show of age, years of service and employment status influential in informing incompleteness medical record file to the medics. The educational background of inpatient medical records clerk not DIII Medical Records and has never participated in the training of medical records. There are officers who are not satisfied with the received overtime pay as deemed incompatible with the duties and responsibilities. There is the cost of maintenance of existing facilities and infrastructure. The need for renewal of Standard operating procedure which provides an explanation of the procedure in detail and detailed activities. Computers as a supporting tool is not functioning properly and need additional room air conditioning facility medical records section so that officers are more comfortable in doing his job.
