Abstract

Based on preliminary studies conducted by interview and observation of the head of the medical records at the University of Muhammadiyah Jember Suherman Clinic on March 28, 2013, the patient registration system and file storage of medical records in Clinic DR. M.Suherman Muhammadiyah University of Jember still using manual systems, making long in search of data, new patient registration and recording system reporting. Based on the above issues and interviews with medical record officer at the time of the preliminary study, obtained information that Muhammadiyah University of Jember DR.M.Suherman Clinical Information System Requires General Poly to improve services in the Public Poli and minimize the loss of medical record file so that researchers develop final project titled "Design of Information Systems General Poly clinic dr. Suherman the waterfall method ". The purpose of this study is Making information systems Umumdi Poly Clinic DR.M.Suherman Muhammadiyah University of Jember. As per the analysis conducted on dr.Suherman Created Clinical Information Systems dr.M.Suherman outpatient clinic using the Waterfall Method menu: Registration of patients, Poli General Examination, Pharmacy, Payment, And this disease. System report has 5 types of access rights the registration officer, admin, doctor, pharmacy officers and officers of payment. With a system of this outpatient information, services such as patient registration, data search, data input checks, drug prescription process, as well as the payments become more efficient report generation and data security is guaranteed
