Abstract

Depreciation of medical records file is an activity of reduction of medical record file from storage shelf. Destruction of medical records file is a process of physical destruction of medical record archive that has ended function and useful value. Implementation of shrinkage and destruction of inactive medical records document at Maesan Bondowoso Health Center still not according to standard and regulation. This is due to the limited information obtained by the executing officers regarding the depreciation and destruction of inactive medical records document, not yet compiled by the SPO and work instructions, the implementer does not make tables of inactive medical records inquiry list, and limited storage room of inactive medical record files. This study aims to analyze the system implementation of shrinkage and destruction of medical record documents to find an alternative problem solving. Qualitative research methods with the concept of system analysis using the 5M management element approach (Man, Method, Money, Material, Machine). Methods of data collection using interviews and observation. The result of this research is an alternative of problem solving related to the implementation of depreciation and destruction of inactive medical record document at Maesan Bondowoso Health Center which is evaluation of medical record management including depreciation and annihilation periodically every 3 months by management, SPO preparation specially depreciation and destruction of document Inactive medical records and the preparation of job descriptions, undertake document deletions to be destroyed and make news of annihilation events plus by changing methods or devising strategies for depreciation and destruction to make it easier and faster, and providing separation barriers to current document storage space, Separate active and inactive storage.
