Abstract
Distribution of the medical record document is an activity of distributing the medical record document to the polyclinic after patient registration. The main problem in the medical record is the delay in the distribution of outpatient medical record document. The time limit for distribution of outpatient medical record document at RSU Dr. H. Koesnadi Bondowoso is ≤ 10 minutes after patient registration. The delay data conducted in the initial survey in November 2019 is reaching 53,5%. The purpose of this research is to develop a strategy to fix the late distribution of outpatient medical records at RSU dr. H. Koesnadi Bondowoso Hospital. This research method used qualitative Action Research. The method to collect data ware using in-depth interviews, observation, and brainstorming. The result of this research was to know the delayed data caused by lack of knowledge about SOP by the officer, lack of officer discipline, no special officer on the distribution of medical record, didn't have a tracer on the storage rack, officer filling didn't even get a reward, and didn't ever do socialization about medical SOP. From the causes above, RSU dr. H. Koesnadi Bondowoso Hospital must revise the SOP and do socialization on their officer
