Abstract
In the Mojoagung Public Health Center (PHC), the inpatient medical record files were piled up in cardboard. Filling shelf is less used and unorderly the filling system as well cause a buildup of medical record files. The buildup of medical record files is cause the time of medical record distribution more than standard (15 minutes). The purpose of this research is to identify the factors that cause the buildup based on the 5M components (man, money, method, materials, machine, to determine and analyz the main problems causing the buildup of medical record files. This is a qualitative research used interrelationship diagram method. The research subjects were coordinator of medical record, medical record officer, outpatient coordinator, coordinator of administration unit. The data were collected by interview, observation and Focus Group Discussion. The research result showed that The root cause of medical record build is lack of knowledge of officers related to filling. Another potential factor there was no did training related to medical record staffs, the filling implementation does not match the storage SOP, there is no budget planning for training. Solution to overcome this problem is by providing additional knowledge to non-graduate medical record staff by the medical records coordinator, making filling SOP and socializing the SOP after conducting retention for optimizing the use of storage shelf
