Abstract

The problem related to the completion of informed consent forms found at RSUD Asembagus is the presence of 94 forms that are still not 100% fully completed. The objective of this research is to analyze the factors causing the incompleteness of informed consent forms based on the factors of ability, motivation, and opportunity. This research uses a qualitative method with data collection techniques including interviews, observations, documentation, USG, and brainstorming. The subjects of this research are 2 doctors, 2 nurses, the head of medical records, and 2 medical records staff. The data analysis techniques in this research consist of data reduction, data presentation, conclusion drawing, and problem priority determination. The results of the research show three main priority problems about the factors that contributing to the incompleteness of Informed Consent forms ability factor (lack of staff knowledge about the implementation of informed consent), motivation factor (lack of punishment), and motivation factor (lack of reward). The solutions that can be implemented include providing training to all staff related to the implementation of informed consent, giving punishment for poor performance resulting in incomplete informed consent forms, and providing rewards for complete and proper completion of informed consent forms.
