Abstract

The emergency medical record form at RSUD Balung Jember in the management process related to the examination of emergency patients is done manually. The impact of the incomplete emergency medical record form at RSUD Balung will cause discontinuity of patient information, resulting in errors of doctors and nurses in handling patients. Besides, it will also have an impact on the quality of medical records, which has decreased due to incomplete medical record files so that the patient data obtained is inaccurate and not on time in filling in the emergency room files. This research aimed to design and create electronic forms of emergency medical records at RSUD Balung Jember using the waterfall method. Data collected by using interviews, observationand documentation. This system was designed using a flowchart system, Context Diagrams, Data Flow Diagrams, Entity Relationship Digram and implementing programs using Notepad++. This research is an electronic form of emergency medical records to facilitate officers in processing medical record files related to patient examination records.
