Abstract

Services and recording of medical records for patients as well as reports at the Independent Practice of General Practitioners dr. Alfi Yudisianto Jember was done manually which caused problems, the problem was duplication of medical record numbers, incomplete diagnoses, old patient medical record sheets that were not found and doctor's writings that were not clear. Problems with manual medical records can have an impact on patient services that cannot be optimal and the work of officers who are inefficient and limited. The purpose of the researchers is to design and manufacture web-based electronic medical records at the Independent Practice of General Practitioners dr. Alfi Yudisianto Jember who can overcome existing problems. The system development method uses the Waterfall method which has stages consisting of defining requirements, designing software systems, implementing and testing units, and integrating and testing systems. Techniques for collecting data using interviews, observation and documentation. The results of the research that has been carried out are an electronic medical record system that can record data on officers, doctors, patients, diagnoses, actions, drugs, visit registration, medical records, drug services and payments as well as patient visit reports, top 10 diseases, top 10 drugs, drug use, and payments. The electronic medical record system can print medical identity cards, medical resumes and reports. Users of this system are front officers, doctors, drug officers, medical record officers, owners of practice places and patients who can view their examination history
