Abstract

This report discusses the analysis of medical record retention and destruction based on the 5M elements (Man, Money, Method, Material, and Machine) at RSUP Prof. Dr. I.G.N.G. Ngoerah Denpasar. The analysis was conducted through observation, interviews, and documentation to identify obstacles in the implementation of medical record retention and destruction activities. The results indicate that the process still faces several challenges, including the absence of dedicated personnel, a specific budget, a valid Standard Operating Procedure (SOP), and supporting facilities and applications for data processing. As an improvement measure, recommendations were developed to revise the retention and destruction SOP and adjust the medical record destruction submission form to support a more systematic and structured implementation of these activities.
