Abstract
The Medical Certificate of Cause of Death (MCCD) is a document containing the sequence of causes of death in accordance with World Health Organization (WHO) standards to produce accurate mortality data. However, Wijayakusuma Hospital Lumajang has not yet implemented the MCCD and still uses the standard Death Certificate (SKK), resulting in cause-of-death documentation that does not meet standard requirements. This study aims to analyze the readiness to implement the MCCD at Wijayakusuma Hospital Lumajang based on the 5M management framework (Man, Money, Method, Material, and Machine). This study utilized a qualitative research design with data collection techniques including written tests, interviews, observations, and documentation. The subjects of this study were the Head of Medical Records, two physicians, and two reporting staff. Based on the problems identified, improvement efforts were formulated using brainstorming. The results showed that under the Man element, staff knowledge regarding MCCD remains insufficient, educational backgrounds align with their professional roles, but none of the informants have ever attended MCCD training. Under the Money element, the hospital does not yet have a dedicated budget for MCCD implementation. The Method element is not ready due to the absence of Standard Operating Procedures (SOPs) and internal policies. The Material element is also not ready because standard MCCD forms are unavailable, leaving documentation reliant on the basic Death Certificate. Meanwhile, under the Machine element, computers, internet connection, and office supplies are available, but the Mortality Medical Data System (MMDS) is not yet available. Wijayakusuma Hospital Lumajang is not yet ready to implement the MCCD due to unresolved constraints across several 5M elements. Therefore, The hospital needs to conduct in-house training regarding the implementation of the Medical Certificate of Cause of Death (MCCD), allocate a dedicated budget, draft SOPs and internal policies, provide standard WHO-compliant MCCD forms, and implement the MMDS to guide causeof-death determination.
