Abstract

The initial assessment form is the form that is used as the first primary quality indicator of the clinical area to ensure continuity of care, medical support and services and patient safety. Based on the results of the preliminary survey, the percentage of incomplete filling in the initial 93 outpatient assessments in October 2019 - April 2020 was 45%, so it was not in accordance with the Health Service Standards, namely filling out medical record forms by 100%. The purpose of the study was to analyze the factors causing the incomplete filling of the outpatient initial assessment form at the Badean Banyuwangi Public Health Center in 2020. This study was qualitative. The subjects in the study consisted of 7 informants, namely the head of the puskesmas, 3 medical recorders, 2 nurses and a doctor. The method of data collection is by observation, interviews, and documentation with data analysis using USG and Brainstorming methods. The results of the priority problems obtained are the attitude of officers who are still not disciplined in being responsible for filling out forms, interference with available Wi-Fi, re-implementation of SOP socialization, no rewards, and item authentication is not filled. The solutions to these problems are improvements to the initial assessment form items for outpatients, providing motivation and warnings to officers, expanding the range of Wi-Fi signals, limiting devices and adding Wi-Fi network access points, tethering mobile hotspots with puskesmas smartphones, and periodic re-socialization.
