Abstract
General practitioner "X" in the district of Rambipuji Jember which has a number of patient visits an average of 20 people / day, or about 520 people / month starting at 16.00 s./d. 22.00 have difficulty in the retrieval of patient medical records. This is due to the arrangement system medical records of patients who are not sorted by medical record number but the name of the family head and the recording of patient data is still manual resulting in the registration process are not optimal and retrieval of medical records took longer. Utilization of information technology in the form of general practitioner “X” information systems is expected to resolve the problem. The purpose of this research is to design general practitioner "X" information systems in the district of Rambipuji Jember. The type of this research is qualitative with waterfall design method and collecting data through observation, documentation, and interview. Results from this research is a general practitioner “X” information system that was created using Microsoft Visual FoxPro 9.0 to store master data (patient, user, icd, medicine, services), transaction data (patients visits, examinations, medical records, cash register), generate reports (patients visits, 10 biggest disease, the top 10 drug use, income) and facilitate the making of a letter (sick letter, health certificate, reference letter) and the medical card patients.
