Abstract
At Banyuwangi Genteng Regional General Hospital the recording of medical record files that came out and returned using manual system. The medical record unit didn’t make a reporting. Based on MENKES RI No. 129 / Menkes / SK / II / 2008 on Minimum Service Standard of Hospital stated that the time of medical record of outpatient medical record is less than or equal to 10 minutes, while in medical record unit of Banyuwangi Genteng General Hospital more than 10 minutes. The existence of these problems the researcher aims to make the information system control of medical records file at General Hospital of Banyuwangi Genteng District by using waterfall method. The result of making information system of medical record file control at General Hospital of Genteng area is application that can issue / lend and do return and print reporting of medical record data. Designed in the form of Flowchart, Context Diagram System, Data Flow Diagram, and Entity Realationship Diagram. Desktop-based applications using Microsoft Visual Basic 6.0 and MySQL Database. Researchers do testing with Blackbox method. The suggestion given by the researchers is that the application is integrated into a web-based hospital information system.
