Abstract
The medical resume must be complete and briefly accompanied by the name and signature of the physician who cares for the patient and can explain important information about the patient, especially the illness, the examination, and the treatment of patient receives. The incompleteness filling of the medical resumes in dr. M. Suherman Clinic Jember,give low impact to the qualityof medical record, and can not be used as material to establish the material cost of health services. Based on the result of 10 documents of medical record file at dr. M. Suherman Clinic got incompleteness data Review identification 65,71%, review 85% authentication, review important report 52,22% .The purpose of this research is to analyze factors causing incomplete medical resume of inpatient in dr. M. Suherman first quarter of 2017 using PTA (Problem Tree Analysis) method. This type of research is qualitative research. The unit of analysis in this study is the head of clinics, inpatients, and medical record staff. Methods of data collection in this study using interviews, observation and branstorming. The results showed that the cause of incompleteness of medical resume filling based on the main problem that is incomplete medical resume filling inpatient, cause level 1 that is knowledge, attitude, policy, and also facility and infrastructure.
