Abstract

One of the information included in the inpatient medical record is a summary of out-patient discharge summary. Discharge summary or outgoing summary is the health report of the person in charge of the service (DPJP) at the end of hospital or medical care. Completeness in the discharge data summary is very advanced quality of medical record is also good bad service of a hospital. The incomplete incidence on the summary debit sheet at the Dahlia Garing inpatient room reached 55.59%. Of the percentage is still far below. Minimum Hospital Service on the completeness of medical records inpati hospital 100%. The purpose of this study is the factors causing factors incompleteness sheet discahrge summary in space Dahlia Crisp Body General Hospital Tabanan. This type of research is qualitative using data and observation data. The result of research of 4 factors (human, method, machine and material) among other factors man stated the existence of time owned by physician who fill the debit summary sheet of limited, reward and punishment given, lack of training and seminar, method factor express not discipline work from the medical recorder of the assembling department and the lack of optimum Standard Operational Procedure (SOP), the machine factor is the non-optimal facilities and infrastructure, and the material factor which states the type of paper used on the sheet of dicahrge summary is the type of carbonis paper by double sheet or duplication activities that can improve the completeness of the summary discharge sheet and the quality of human resources that includes doctors and medical record officers assembling in training and seminars related to the completeness of medical records
