Abstract

The dental medical record is a systematic documentation of the patient's condition and all actions given to the patient by the dentist. Based on a preliminary survey conducted at the outpatient unit grujugan bondowoso public health center only has one outpatient medical record form which is used for all outpatient polyclinics. The purpose of this study was to design a dental poly medical record form according to user needs based on physical aspects, anatomical aspects, and content aspects, and in accordance with the 2015 ministry of health's dental medical record guide. This type of research is qualitative research with data collection methods of observation, interviews, and brainstorming. The subjects of this study were the head of the health center, dentist, dental nurse, head of medical records, and 2 medical record officers. The result of this research is a dental medical record of dental poly form design with the identification of physical aspects of the form are hvs 80 gram paper, f4 size with landscape orientation, and white paper color with black ink. Anatomy aspects of the form include additions and changes to headings, introduction, instruction, body and close from the previous form. Content aspects of the form include the addition of patient medical data items, odontograms, and supporting attachments in the approval and rejection of dental medical actions. The results of this study are expected to be used as a consideration for submitting the form so it can be used or applied and can help to support the completeness of information on medical records of dental patients at the grujugan bondowoso public health center.
