Abstract

A health service facility is a place for implementing individual and community health service efforts by providing mental health services. One of the activities implemented at the mental health service is recording the assessment of mental disorders. This assessment is recorded on a mental disorder nursing assessment form that must be provided with format according to the rules of the design form and the process of the mental nursing assessment. So, the health service can be provided with integrated, comprehensive and sustainable through the promotive, preventive, curative and rehabilitative services. The Purpose of this research is to design the assessment forms of nursing mental disorders according to the rules of standard design forms and the nursing mental disorders process.The research method is used literature review or reviewing selected articles from academic repositories that were selected using inclusion and exclusion criteria. Results of the research there are 14 selected articles will be reviewed by describing and then comparing the result of previous studies. Selected articles consisted of 7 articles with general form design topics and 7 articles with mental nursing disorders topics.Conclusion of the research is the design of the mental nursing assessment form was designed based on physical aspects, an anatomical aspect, contents aspect, and mental nursing assessment processes based on the reviewed articles, so that resulted in complete and informative form. Research suggestion is necessary to have SOP and evaluation related to form design.
