Abstract
The medical records of acute diarrhea cases on toddler under five at RSU Dr. H Koesnadi Bondowoso are not accurate and complete yet. Such as, unwritten on physical examination for severe dehydration patient, mild dehydration, or non dehydration due to the treatment to be carried out will be different. The main plan was also not written on a column treatment plan which will be implemented in a patient. They anamneses do not have information on the amount of liquids, food and drinks were consumed during diarrhea. The purpose of this study was to analyze about medical qualitative of hospitalized toddler‟s medical record in cases of diarrhea 2015. The method used is qualitative data collection techniques through observation and interviews. The results of this research is still a lot of incomplete record-keeping toddler patients hospitalized with acute diarrhea cases based on the Pedoman Praktek Klinis (PPK) acute diarrhea belonging to RSU Dr. H Koesnadi Bondowoso. Therefore the quality of the medical records of toddler medically acute diarrhea is still not good. Medical qualitative analysis should be conducted once a month so that medical records can be a good guideline for the medical care of patients and required good cooperation to all the medical officer in charge of record-keeping.
