Abstract

In medical record activity to support the implementation of the security and confidentiality of health information in the medical records, the researcher found several problems that doctor recidents that came into the filing room and took himself medical records that is needed, and also the number of returned of medical records were not on time still high. The purpose of this research to review the implementation of the security and confidentiality of health information in the medical record file using fishbone diagrams Dental Hospital Jember University in 2013. The variables in this research are manpower, machines, methods, materials, media, motivation, money. The results of this research are medical recorder are graduated from diploma 3 is educational medical records and health information and understand the SOP on security assurance of health information in the medical records. Using the manual and electronic tracer in ensuring the implementation of the security and confidentiality of medical records. SOP on the securing of medical records that came into force in 2005. Medical records folder is too thin, causing the medical record file can not stand firm, not neat and easily torn/damaged. Although there is a warning sign on the front filing door that’s shown “staff only”, but ignore the rule. The punishment and reward for staff medical records and recidents are not implemmented. Medical records never do budgetin . Hospitals should be able to perform the optimization of the use of tracer is electronic using existing information system, added a punishment dan reward into procedure for medical recorder and users of medical records, and provide a permanent barrier insulation in order to ensure the implementation of the security and confidentiality of health information in the medical records .
