Abstract

Informed consent is the approval of medical action which is given by patient or their close family after they get the complete explanation about medical action that will done toward the patient. One of parameter to certain the quality of health service in the hospital is the completeness of filling up form Informed Consent. The purpose of this research is to certain what factors which cause incomplete of filling up Informed Consent in RSIA Srikandi IBI Jember. The type of research is qualitative by look for the information through the interview to the health staff, for the data validation use triangulation source. The result of research be found some item have majority percent incomplete which marked the doctor 45% (31 form), marked the witness 83% (59 form), and name second witness 87%% (59 form). Cause incomplete because SDM factor that is not discipline the doctor, activity the doctor, and not understand midwife and nurse in filling up informed consent. Hospital it should be done the reward and sanction to increase the discipline of health staff and to revise SOP of informed consent.
