Abstract

A dental medical record is a systematic documentation of a patient's dental health history by means of health care. Based on the preliminary survey, Maesan Bondowoso Community Health Center does not have a special outpatient medical record form for dentistry. During this time using a form that is used to all types of existing poly. The purpose of this research is to conduct the design of outpatient medical record form on dentistry based on user requirement and National Standard of Dental Medical Record 2007 from physical aspect, anatomical aspect and content aspect. The type of research used is descriptive qualitative with data collection method using interview, observation and brainstorming. The result of this research is the design of outpatient medical record form on dentistry based on the physical aspect of paper material using BC 160 gram paper. Anatomical aspect that is there is addition of identity of the community health center. While the content aspect is the addition of items in the patient's identity group, the patient's general condition data group / disease in the patient to note, the odontogram group, the dental care data or the treatment table. Suggestions from the results of this study are expected to be used as a consideration in the planning of proposing an outpatient medical record form on new dentistry for use in Maesan Bondowoso Community Health Center.
