Abstract

Poorly designed forms can result inadequate data collection, documentation becomes long, false information, and another errors. In-patient medical record form at public health center of Tegaldlimo District Banyuwangi Regency there are some non-existent items such as patient diagnostic code, doctor's signature or other responsible health officer and the form is still made manually by the officer so the edge line is not straight. The purpose of this research is to redesign the medical record form of in-patient at public health center of Tegaldlimo District Banyuwangi Regency. This type of research is qualitative with unit analysis approach, where data collection is done by observation, interview and brainstorming. The result of the research is the new design of the in-patient medical record form at public health center of Tegaldlimo District Banyuwangi Regency which is suitable for the user's need. Recommendation for public health center is to implemented the redesign of the inpatient medical record form at the public health center and supported by the design of the map so that the form is maintained.
