Abstract

The activity of recording and reporting system (SP2TP) in Candi Health centre of Sidoarjo District is not maximally because data integration is not avaiable of each unit and activities. The purpose of this research is to make integrated application of recording and reporting activities in health centre. System development method using prototype method. First stage is listen to customer. This stage is done to the informant of the registration officer, medical officer, administrative officer, and officer of activity holder. The second stage is build/revise mock up. It consists of design prototype and coding. Prototype design uses Use Case Diagrams, Class Diagrams, and Sequence Diagrams. Next stage is coding using Java programming language with MySQL database. Last stage is customer test drive mock up which is evaluation phase using blackbox which tested by user. The output of this application is to include monthly report of health centre which fit with SP2TP format. Test results on the user indicate that the application can be used functionally and received by the user.
