Abstract

The medical record unit is responsible for managing patient data into health information useful for decision making. As one of the special hospitals RSIA Srikandi IBI Jember should be able to perform a good medical record management according to the standard, especially in the process of storing medical record files. It is known there is a build up of files that cause misfiles in the storage space, but the hospital does not do the retention. Therefore, it is necessary to analyze factor in retention activity. One tool to determine the cause of the problem is to use the fishbone diagram method. This study aims to determine the factors in the activity of depreciation in RSIA Srikandi IBI Jember with fishbone diagram method. The result of the research shows that the absence of the team of medical record file retention has an impact on the implementation of shrinkage of medical record file, the officer's education has not complied with the specified qualification, the officer has not attended the training, SOP of Medical Record File Retention has not been renewed, the absence of budget for retention activity become wrong one inhibiting the retention of medical record files, and the unavailability of inactive medical record storage shelves and the unrecorded shrinking of medical record files. It can be concluded that the 5M factor of man, method, money, material and machine become factor in shrinking medical record file. It is expected that the renewal of SOP of Medical Record File Retention can be maximized.
