Abstract

Every health service has been given to the patient, it should be kept in the medical record. The medical records of hospitalized patients in hospitals must be kept at least for five years. The hospital has a policy on the retention period / storage of documents, data and information. Mother and Child Hospital (RSIA) Muhammadiyah Probolinggo has been established since 1977. For almost 40 years of existence, the retention and exterminaton process is done once. It is caused there is the difficulty implementation of retention and exterminaton of medical records manually. This research’s purpose by design and create a system of information retention and exterminaton of medical records. This research is a qualitative research with waterfall development method. The collection of data through the observation, interviews and focus group discussion to seven respondents, including heads of medical records and filing clerk. The results of the research is a requirement system of functional and non-functional information system retention and exterminaton of medical records to be made. Then the necessity is transformed into a form of system design that is system flowchart, context diagrams, data flow diagrams and entity relationship diagram. The design then transformed into machine language using the PHP programming language and MySQL database. The test results show that the information system made in accordance with the necessity and there are no errors. The information system can facilitate the implementation of retention and exterminaton of medical records manually and improve security of data storage. Furthermore, it would be better if the information system that has been created to be integrated with the hospital information system to increase the utilization of data.
