Abstract

Medical record service sistem are is a system that organize form, note and report in which coordination to served the document who needed by the hospital management and apply for the patient healing, then in a variety of health recovery efforts there should be consent from the patient on the basis of information from doctor at the hospital called informed consent. This research using qualitative methods. The preface survey result from the Kaliwates of Jember head of the medical record, researcher found some complain often appear after the sectio caesarea in cause of the minimum comprehension and the understanding about informed consent the doctor convey to the patient. This research aim to find out the doctor and sectio caesarea patient knowledge of extend and taking and also to understand utmost the contents and purpose of informed consent and to checking the completeness of patient identify filling of the medical record informed consent. This study uses the unit of analysis is the medical record contains parts poly. Instrument used is check list observation and interview orientation. The result found that the doctor convey informed consent appropriate with the standard but, unconscious doctor used language which is not understand easily by the patient so, not all of the patient be able to understand the information that the doctor gived at the result patient/vice did not fill the data in the informed consent letter. Advice can be given in this study is that the evaluation and rechecking in the process of delivering information to the patient, the patient’s understanding and completeness of the sheet informed consent before their consent to the action to be performed by a physician.
