Abstract

The one criteria of a good hospital is implementing the right medical record management includes numbering system that become was conducted showed that beginning for service’s sequence toward the patients. The preliminary study in dr. R Soedarsono, the double numbering was still found, there were 15 duplication of medical record numbering, even there was a patient has 4 medical record numbering that always found on every month. The aim of this research is to evaluate the implementation of Standard Operating Procedur in medical record numbering with CIPP (Context,Input,Process,Product) method on outpatient unit of dr R Soedarsono Hospital. It us a qualitative research. The data collection was from interview to the head of medical record and 5 registration officers, observation, documentation, and brainstorming. The result of research show that the tracer will be reactivated on the decision planning, 1x24 hour DRM restitution in every polyclinic, giving notification on SIMRS on registered patient, giving medical recording number on patient identity card, determine employee minimum recruitment D-III Rekam Medis, rivising the old SOP. The result of planning decision have been conducted in a month is success but the improvement of manual tracer still needed. The suggesstion of the study are supplying electronic tracer, supplying medical requirement banner to bring KIB, minimum requirement D-III Rekam Medis for the employee, training or seminar and giving SIMRS notification on registered patient
