Abstract

Baladhika Husada Jember Hospital is a military hospital with cases of carcinoma mammae included in the top 10 diseases in the first quarter of 2018. The medical record of carcinoma mammae patients at Baladhika Husada Jember Hospital contains incomplete and accurate information. This study has purpose to analyze the quantitative and qualitative administrative medical records of hospitalization in patients with carcinoma mammae at Baladhika Husada Jember Hospital. This research type is qualitative methods with observation and iterviews. The subjects of this research is a oncology specialists, cancer nurses, head of medical record, and inpatient registration officers. The sample are 87 inpatient medical record of carcinoma mammae period of the first quarter in 2018. Observation results found 29% incompleteness of social identity, 6% proof of record, 89% official signatures, 42% recording procedure. There is no clarity of problems and conditions 21%, inconsistent input 28%, informed consent 56% incomplete signatures and bright names of patients or guardians of patients, review of recordings 49% the condition of writing is difficult to read. The interview results is cause of incompleteness were not yet carried out routine and detailed completeness analyzes of medical records of carcinoma mammae. Advice is to do socialization on the importance of completing the filling of medical records and conduct analysis and evaluation of the completeness of the medical record file on a monthly basis to assess and improve services that have not been optimal.
