Abstract
Hospital as medical record service provider can responsible for all information of patient health care. The documentation of patient health care can uses to valuate of health care quality for cases which to be the highest morbidity rate. RSD Balung Jember has LBW cases always included in the top 10 causes of morbidity rate annually. However, there were many neonatal medical record in RSD Balung with incomplete and inaccurate of information. So, this study has purpose to analyze of qualitative medical information in the inpatients medical records of LBW cases in RSD Balung Jember 2016. This research type is qualitative methods with observation and interviews. The sample are 21 inpatient medical record of LBW case in 2015, and the subject of this research is a pediatrician, head nurse of perinatology and head of maternity room. The output of observation for sample of medical records are still incomplete information of LBW cases. The interview result is cause of incompleteness as indiscipline factor from personnel service providers, design form medical record are not appropriate with the services provider’s needed. Researcher advise to create the standard operating procedures (SOP’s) or guidelines for LBW health care, as well as RSD Balung can be applicable the medical qualitative analysis who can assess the quality of service and can to repair the health care,.
