Abstract

Based on the theory that researchers read when implementing a new information system is necessary to do an evaluation of the information system. There are 5 types of evaluation stages one of which is operational evaluation-1 is the evaluation stage after Implementation Review conducted. This study aimed to evaluate the electronic medical record information system that has been implemented. Measurements were performed using the method PIECES and CCHIT. This measurement is based on the officer's perception of the six dimensions of information systems and the identification of determinants of program success. Based on these results, it was found that the results of the evaluation of the electronic medical record information system in UPT. Polyclinic Polytechnic of Jember based perception officer stated well.And observations on aspects of CCHIT to determine the success of the program known enough to meet the criteria. From these results it can be concluded that the evaluation results can be expressed either. However, the need for further development of the program that need to be evaluated every month to see the correspondence between the planning of service activities, programs need to be developed that is integrated into a polytechnic institute muddy country, needs a coordinator needs to be able to prepare in planning program development (upgrade), programs need to be revised in order to meet the standards of CCHIT.
