Abstract
Chemotherapy is treatment that use anticancer drug (sitostatica) to kill cancer cell. Chemotherapy is sustainable treatment therefore chemotherapy form brought officer to easier documentation for subsequent treatment, it cause chemotherapy form patient often broken or lost. This research purpose to design and manufacture chemotherapy information system at dr. H. Koesnadi General Hospital in Bondowoso which can record chemotherapy patient electronically. This study use waterfall method, research type is qualitative research. Collecting data use observation, interview, documentation and brainstorming. Chemotherapy information system design using programming language microsoft visual foxpro 9.0, chemotherapy information system design refers to chemotherapy form at dr. H Koesnadi General Hospital in Bondowoso. This research conducted at dr. H Koesnadi General Hospital in Bondowoso. The result is chemotherapy information system at dr. H Koesnadi General Hospital in Bondowoso consist of master data includes: data patient, drug, ICD10 and user: data transaction includes: patient registration, protocol doctor, nurse protocol, assessment chemotherapy patient, record drug and schedule chemotherapy: the report includes: visit patient report, drug report, chemotherapy schedule, 10 diseasse report and chemotherapy reminder.
