Abstract
Dr. R. Soedarsono General Hospital in Pasuruan regarding form in out summary and medical resume manually, on both the form used as a front and back cover inpatient medical record file so that the confidentiality on both the form and become less awake. In addition to the summary forms in out which made the front cover of medical record file hospitalization is also coupled with sheets consul and radiology form, whereas in the medical resumes form serve as the back cover is also coupled with the laboratory results of the patient form. This research purposes was to design a sign out the summary forms and electronic medical resume. This type of research is qualitative using waterfall design. Stages in this model include analysis, design, coding and testing. The results of this study are in and out of the summary forms and electronic medical resume It is hoped their electronic form can ensure the confidentiality on both the form and besides it can service speed to patients, and can minimize the occurrence of errors in writing. As well as a summary form in out system and medical resume can be integrated with the SIMRS.
