Abstract
Medical record is a file that contains records and documents include identification of patient, examination result, treatment has been given, as well as actions and other services that have been given to patients. The purpose of this study is to examine the relationship of knowledge, attitudes and behavior towards completeness sheets doctor informed consent in RSU Bhakti Husada Krikilan. This research used in this research is quantitative or correlation analytic method with cross sectional approach. Statistic test used spearmen correlation with α = 0.05. Based on the results, more than half of the implementation of the charging sheet of informed consent by doctors incomplete (96%). The results showed that there was a significant relationship between knowledge, attitudes and behaviors with sheet completeness of informed consent in RSU Bhakti Husada Krikilan. Correlation value p = 0.032, for the connection of knowledge with a complete sheet of informed consent and p = 0.002, for relations with the attitude of informed consent completeness sheet and p = 0.044, for completeness sheet behavioral relationships with informed consent. The hospital should provide training and seminars relating to the completeness of the recording sheet informed consent, because a good understanding of physicians will influence the knowledge, attitudes and behavior him in RSU Bhakti Husada Krikilan.
