In this book, we attempt to present the current state of quality and safety in neurosurgery. We divide this book into three main sections: (1) the importance of quality and safety in neurosurgery; (2) measuring and improving performance; and (3) the practice of quality and safety in neurosurgery. In the first section, the importance of quality and safety within neuro�surgery is examined. The historical role organized neurosurgery has played in the last 10–15 years to advance patient safety in neurosurgical practice is discussed. The role of reconfigured residency training, simula�tion training, and competency evaluation in teaching safety science and principles to a new generation of neurosurgeons is considered. The impor�tant role of outcomes reporting and the pursuit of quality improvement efforts to promote patient safety are highlighted. Volume-outcome rela�tionships and the issues of regionalization of neurosurgical care are explored. The push to create a culture of safety within neurological sur�gery, and the role of professional societies and journals in setting the stan�dards for scientific exchange of knowledge are discussed. Finally, the need of all practicing neurosurgeons to be involved in a continuous, ongoing process of improving the results of their clinical interactions with patients and process improvement is noted. The second section deals with the challenging issues pertaining to accu�rately measuring and improving performance. The methods for the devel�opment of a neurosurgical quality and safety program to measure and analyze metrics related to complications and outcomes are reviewed. The important and growing roles of clinical registries and outcome data to guide clinical decisions and use of these registries for benchmarking, value-based purchasing, disease characterization, comparative effective�ness research and stimulation for quality improvement efforts is reviewed.
