Abstract
Balung Regional Hospital is a class C hospital belonging to Jember district government, equipped with special outpatient / polyclinic facilities, inpatient facilities and supported by medical support facilities also supported by pharmacy installation and supported by central surgery installation facility / operating room. The purpose of this research is to analyze the priority The cause is not implemented shrinkage and destruction of medical record files in RSD Balung Jember.The method used in this study is the method of ultrasound (Urgency, Seriousness, Growth) with the root withdrawal of the problem using 5 elements of management. The type of this research is qualitative research with data collection technique using interview, observation, and brainstorming. The results of this study found that the priority cause is not done depreciation and destruction of medical records file is in terms of Man which includes education, knowledge and training, so the hospital is expected to create a special team for depreciation and destruction activities so that the implementation of depreciation activities and destruction of medical records file can run well.
