Abstract

Depkes RI in permenkes2008, Medical Record is a file containing records and documents about patient identity, examination, treatment, action and otherservices tha have been give to the patient. Based on the preliminary survey of Ajung Jember Clinic, the average number of impatient of the medical record file, in January there are 5 files that are not complete and moon 4 incomplete files from here the reseachers want to solve the problem of incomplete file. The purpose of this study is to analyze the factors causing incomplete medical records at the clinic Nusa Medika Ajung jember 2017. This type of reseach is qualitve by using data collection method of observasi, interview, the result of fishbone research (man,method,machine,material,money) the caused the incomplete filing of medical record file is the ability of officers the need to be improved again, the absence of Standart Operasional Prosedur (SOP) from the clinik related to the implementation of filling in patient medical records, the availability of supporting tools such as stationery can assist in filling the file, the design of the should be easily understood by the office, the budget of the clinic such as the salary of bonus employes and other expenses must be balanced.
