Abstract

The activity of medical consultation/education in hospitalization clinic of DR. M. Suherman as one of chronic disease application activities (Prolanis) is not done routinely because the doctors are not available to do medical consultation/ education. Diabetes Mellitus and Hypertension are chronic disease that can be controlled by doing a proper diet. Knowledge about the proper diet pattern for the members of prolanis cannot be acquired because the implementation of medical consultation/education is rarely done. The objective of this research was to make web-based diet therapy application for the members of prolanis. The method of system development used in this research is waterfall method. The need analysis step was done by collecting information from a doctor and nutritionist. The application design used flowchart document, DFD, and ERD. The next step was coding which was done by using assembly language of PHP 5 and Mysql database. The application try-out was black box method by user of the members of prolanis, doctor, nutritionist, and admin. The application output included the calculation result of the prolanis member diet therapy, which was a menu of Diabetes Mellitus diet therapy, Hypertension, Diabetes Mellitus and Hypertension, renewed data of diet therapy menu, file of calculation result, file of printable suggestion of the doctor, and visite Prolanis of report. The result of the try-out by blackbox method on four users showed that the application functionally can be used accurately, easily, and well accepted by the users
