Abstract

Community health center is the first level of the health services unit and the forefront of the health care system that is adapted to the conditions, needs, demands, capabilities and innovations as well as local government policy. Generally the process of logging on clinics are still done manually so it takes a long time and the paper used does little to archive patient. The purpose of this research is to design a Program of information system Web-based Outpatient to ease the process. Manufacture and design of information system of this Webbased Outpatient method waterfall include : (1). Requirement Analysis and Defination, (2). System and Software Design, (3). Implementation and Unit Testing, (4). Integration and System Testing. This research resulted in the Outpatient information system that can be used in the creation of web-based medical record.
