Abstract

Polyclinic dr. TRIWIRANTO a public health service agencies located in Earth Mangli Permai Housing Block A - 07 which provide outpatient services to the more than 5100 people. Polyclinic which has considerable number of patients to be able to provide the best possible service in the process of medical examination as well as in the processing of patient data and medical records of patients. Computerized data processing is necessary for each agency. In addition to data storage security, it will also help every officer in doing his job. Processing of medical data of patients in Polyclinic dr. TRIWIRANTO still done manually by noting in a book of registration of patient visits and noted a history of patient's medical record on a card medical record so frequent problem of human error such as a typing error in the name, the recording history of patient medical records, preparing reports that are sometimes less accurate and medical record data search takes a long time because there is no medical card patients. Therefore, it will be made the design of an information system polyclinics to assist officers in the process of work, especially in administrative services and computerized data processing to be more efficient and more leverage as well as data storage media history of the patient's medical records are computerized. Clinic Information System will be designed to use desktop-based programming language will then be developed with the Microsoft Visual Studio software and the MySQL DBMS.
