Abstract

Completeness of medical record file and correct diagnosis of disease will affect the financing system of services, especially in patients BPJS. Against the background of the patient's complete medical record file of diarrhea in RSUD Abdoer Rahem Situbondo are still many experienced incomplete records and the contents of the medical record file is empty, thus might have an impact on health care financing system by the hospital. The purpose of this study to analyze qualitatively completeness of medical record file of diarrhea in hospital patients Abdoer Rahem Situbondo first quarter of 2015. This type of research is qualitative descriptive with cross sectional approach. This study population numbered 177 medical record file, and the sample of 50 medical record file. The result is that the accuracy Award codefication appropriate management of diarrhea file file numbered 104, while the number of diarrhea incompatible file amounted to 19 medical record file. Codefication error caused by the clerk did not know codefication K29.1. Therefore, it should be socialized and seminars regarding the accuracy Award codefication appropriate disease management.
