Abstract

A form must be designed to fulfill its purpose. Hospital Perkebunan Jember (Jember Clinic) have a new form for outpatients. This form is applied only began in February and there has been no evaluation. The form decreased levels of completeness. The purpose of this research to evaluation the new form outpatient methods of fishbone. This type of research is descriptive qualitative method of observation, interviews, and a brainstrorming. The results of this research limited time officers filling in the forms of new outpatient caused by too many items and there is no socialization of the form. Service recording not only did manually, but it is also did electronically. The number of items that must be loaded form is not supported by state of the space provided and there are similarities information obtained thereby causing incompleteness as well as the convenience of users. No updates to the Standard Operational Procedure (SOP) Related Procedures Examination Filling Card Outpatient.Budgeting for the form used to do every year, but the procurement accordance with the needs of the hospital. Priority of the problem in a new form based outpatient brainstrorming is influenced by the incompleteness of filling the fom in terms of desain and form improvement efforts have been made as the user desires. Suggestion of improved yields outpatient new design form is expected to be applied and carried out reform so that charging Standard Operational Procedure (SOP) and disseminated to users, thereby complete data and information obtained from the form in acccordance withnthe required maximum users.
