Abstract

A community health center or what is commonly referred to as a community health center is a first-class health facility that provides health efforts for individuals and the community, prioritizing early-level preventive and promotive efforts. All patients who visit the puskesmas will be written according to their respective identities. All complaints and medical actions by medical personnel as well as medication given to patients will be written in the administration section. Everything is written in the patient's file which is called the patient's medical record file. medical. Patient medical records at the community health center will be stored for a minimum of two years. At the Dawarblandong community health center, retention and destruction of medical records is still implemented manually. The research aims to create and design information on retention and destruction of medical records. Researchers used qualitative research using the waterfall method. Data was collected through observation, interviews and brainstorming with the head of medical records. This need is translated into a system design in the form of a system Flowchart, Context Diagram, data flow diagram and Entity Relationship Diagram. This design is translated into the PHP programming language and MySQL database. The existence of this information system can facilitate the implementation of destruction and retention of medical records, and increase data storage security
