Abstract

ArdhianMalikul Aziz Study Program of Medical Record D-IV Health Department ABSTRACT The development of information technology can make it easier for technology users to receive the information needed quickly, especially in the health sector,medical record is very important in health services, because medical records are used as a source of information both social data and patient medical data and all forms of medical services that are given to patients by health workers. This research’s purpose by design and create a system of information retention of medical record. This research’s is a qualitative research with waterfall development method. The collection of data through the observation, interviews and documentation to filing clerk and heads of medical record. The results of the research is a requirement system of functional and non functional information system retention of medical record to be made. Then the necessity is transformed into a form of system design that is system flowchart, context diagram, data flow diagram and entity relationship diagram. The design transform into visual basic.net program and mysql database. The test results show that the information system made in accordance with the necessity and there are no errorsThe information system can facilitate the implementation of retention and of medical record in puskesmasbanjarsengon.
