Abstract

Arjasa Jember Health Center is a B accredited health center and the only health center in the Arjasa sub-district area. The Arjasa Community Health Center has not yet implemented SIMPUS, so officers have difficulty processing reports from the Community Health Center to the Health Service. Arjasa Health Center has registration data in the form of Microsoft Excel, this system is expected to be able to record and report at Arjasa Health Center. The aim of this research is to design an outpatient registration application at the Arjasa Community Health Center using the Waterfall method as input which is expected to produce accurate information. The system design method uses the waterfall method. Data collection techniques use observation, interviews and documentation. The results of this research are that the outpatient registration application for reporting to the Jember Health Service makes it easier for the reporting process to the Health Service and supports the quality of services at the Arjasa Health Center.
