Abstract

The delay in returning outpatient medical records at RSPAD Gatot Soebroto still frequently occurs and can hinder subsequent service processes. This study aims to analyze the causes of these delays, including factors such as the lack of training for medical record and polyclinic staff, as well as the ineffective use of the expedition book. The research data were obtained through document studies and interviews with filing staff. The results show that in 2022, 779 medical records were returned late, particularly in the vascular surgery, dermatology, and neurosurgery clinics. Training and the reimplementation of the expedition book, as well as warnings for polyclinics that delay the return of records, are needed to improve this process. The addition of a return time standard in the SOP is expected to help staff perform their duties according to the established procedures.
