Abstract

Based on preliminary studies it is known that in hospitals cockscomb in the manufacture of a Visum et Repertum there are aspects of the form not in accordance with the physical aspects of the bag used with thin materials, in the anatomical aspect there is no heading or head of the form and in the content aspect there is no specific examination information for the anatomical location. The Visum Et Repertum will be true evidence and in accordance with the actual conditions so that the judicial process can run fairly. This study aims to redesign the postform mortem et Repertum at the Balung Jember Regional Hospital. The purpose of this study was to design theform Visum et Reperum at the Balung Jember Regional Hospital. This type of research is qualitative research with research subjects including emergency room doctors, medical record officers, and emergency room administrators. Data collection techniques in this study using interviews, observation and documentation. The results of this study were seen from the physical aspect of the Visum et Repertum form using white paper with a size of F4 70 grams (33 cm and 21.5 cm) in portrait form. Judging from the anatomical aspect of theform, the Visum et repertumheading includes the logo, name of the agency, agency address, agency telephone number, agency website, agency email and the title of the form, for the introduction it is represented by the title. An examination of the Visum et Repertum form data is added consisting of perimtaan letter Visum et Repertum ,reports, Visum et Repertum cover and attachment inspection. Suggestions from researchers that the design results of theform are Visum et Repertum expected to be a reference for the Visum et Repertum design at the Balung Regional Hospital, Jember.
